Biography:

Chandran Rajagopal has completed his Graduation from the Madras Medical College, University of Madras in 1972. After his Graduation from MMC, he served in the Malaysian Health Service in the fields of Psychiatry and General Surgery. He continued his training in Dermatological Surgery in US with the American Academy of Aesthetic and Restorative Surgery in New Orleans, Louisiana under Professor George Farber and became a Fellow of the Academy and later as an International Fellow of the American Academy of Dermatologists.

Abstract:

Electrosurgery refers to four different methods: Electrocoagulation, electrodesiccation, electrofulguration and electrosection using high frequency to perform various procedures in dermatology and aesthetic surgery. Aim of this study is to ascertain the mode of operation in performing several procedures safely and successfully involving high frequency alternating current, which converts to heat by resistance as it passes through the tissue. The result of heat buildup within the tissue which results in thermal tissue damage, thus enabling several procedures, these modalities are commonly used for hemostasis, debulking procedures such as rhinophyma excision and treatment of benign and malignant skin conditions ranging from acrochordons (skin tags) to Basal Cell Carcinoma (BCC). Electrosurgery is a surgical technique which involves sequential curettage (scraping) of a skin lesion followed by electrodesiccation (a form of electronic cautery). The sequence is generally repeated 3 or 4 times. It is an alternative to more invasive surgery. It is used to treat benign and superficial malignant skin lesions including but not limited to seborrheic keratosis, may include observation, excision and curettage of actinic keratosis, nodular and superficial basal cell carcinoma, squamous cell carcinoma in situ. Risks include discomfort, pain, bleeding, burns, electric shock, recurrence, conversion to excision, infection, changes in pigmentation and scarring. Adequate training and understanding of the electrosurgical unit is very essential to the physician using this equipment to perform safe and successful surgery

Kong Chee Kwan has double is a General and Plastic Surgeon. He started his career in general surgery in 2004. He served as a General Surgeon at the Ministry of Health Hospitals before underwent sub-specialist training and board certified as a Plastic and Reconstructive Surgeon. Consequently, University Malaya appointed him as a Senior Lecturer and Clinical Specialist in the Department of Surgery. He has trained many general surgeons as well as plastic surgeons. He has published numerous papers in reputed journals and has been serving as an Editorial Board Member of Malaysia Medical Journal.

Abstract:

The principle of facial aesthetic units is important in designing local flaps for facial reconstruction. Local flaps are routinely utilized for skin lesions that are too big to be closed primarily. The ideal local flap for reconstruction is the one that can be designed within the same aesthetic unit as that containing the lesion. Scars are best concealed by placing incisions along aesthetic margins. When a defect involves two or more aesthetic units, it is better to compartmentalize the reconstruction. Each skin flaps are planned to reconstruct the separate components of the defect that are located within separate aesthetic units. This may provide similarity of skin quality but, more essentially, places scars in the aesthetic margins. It is often beneficial to extend the primary defect by increasing the defect to an aesthetic margin or even to extend the defect to occupy an entire aesthetic unit. Reconstruction of the defect with a local flap will then position a border of the flap in an aesthetic unit for improved scar concealment.

Abstract:

Involving using of human stem cells in Androgenetic Alopecia (AGA) holds a great promise. There are numerous sources of autologous human stem cells being used for the treatment for AGA. A recent new discovery source of autologous human stem cells is harvested from human hair follicles. Hair follicles are known to contain a well-characterized niche for adult stem cells: Mesenchymal stem cells in the dermal sheath and the bulge, which contains epithelial and melanocytic stem cells. Stem cells in the hair bulge, a clearly demarcated structure within the lower permanent portion of hair follicles, can generate the interfollicular epidermis, hair follicle structures and sebaceous glands. A new method to isolate human adult stem cells is by mechanical centrifugation of punch biopsy from human hair follicles without culture condition. Area of punch biopsy at the mastoid process along the hair line demarcation bilaterally. Human Follicle Stem Cells (HFSCs) is used to improve the hair density in patients affected by Androgenetic Alopecia in stage 2-5 as determined by the Norwood-Hamilton classification scale and few cases of alopecia areata. It has shown that the isolated cells are capable to improve the hair density in patients affected by androgenetic alopecia and in some cases of alopecia areata.

Abstract:

Statement of the Problem: People who are retired early at 58-65 year old depending on country brings more harm than good the patient, society and nation. They are difficult to get re-employment, couldn't meet the rising living expenses in the inflationary world, also doubled up with many illnesses that require more budget for the hospital visit and medication. This is compounded with higher longevity with expected life expectancy of around 80+ years. These 20 over extra years of living post-retirement incurred a lot of financial burden challenges personally, on the society and national economic level with inadequate worker leaving the workforce or skill mismatch. This problem faces mankind for the first time never seen before. Methodology & Theoretical Orientation: Utilizing a subjective analysis reviewing books, articles, clinical history taking, charity function observation & interview, and focus group to gather data. Using the security framework of the social sciences to understand the interaction between the ageing patient and the socioeconomic factor relationship and context. Findings: The post-retirement patient group were actively seeking new employment despite skill mismatch to find their personal worth, meeting financial obligation, relieving physical, social, mental and spiritual boredom. Those left unemployed faces mental stress, judgment and memory deterioration faster, physical body regression, withdrawn and ages faster. One interesting finding is that more would like to do charity work & attend religious prayer. Society tends to face GDP decline without government policy interventional help. Conclusion & Significance: Retired patient faces tremendous burden on financial, stress and ages faster with less priority given to health consequently. Therefore, they end up with severe medical complication. Timely and adequate support from the medical, socio and government intervention will improve their employment, health and economic security. Recommendation: All need to start early with financial planning to allow adequate buffer zone cushioning the impact after retirement. They need to be health savvy to maintain healthy body, or comply with medication to avoid complication insuring higher cost. Social welfare and geriatric support and peer group support are essential.

Many researchers believe that over the next two decades, stem cells will change medicine forever, extend life and potentially save our life! Abstract:

Introduction & Objectives: Scarring is one of the most common and difficult to treat sequelae of acne. Acne scars have a negative impact on the quality of life of those affected. The objective of this study was to compare the effects, number of treatments required and side effects of fractional CO2 laser and fractional Microneedle Radiofrequency (MnRF) in treatment of acne scars. Materials & Methods: Twenty (20) patients with acne scars and were included in the study, 10 in each group. Patients in each group underwent 4 sessions of either CO2 laser or MnRF one month apart. The therapeutic response to treatment was assessed at each follow up visit and then finally 4 months after the last treatment. Response to treatment was labelled as ‘excellent’ if there was >50% improvement in scar appearance and texture, while 25-50% response and <25% improvement were labelled as ‘good’ and ‘poor’ respectively. Overall satisfaction of the patients and any adverse reactions to the treatment were noted. Results: A significant improvement is acne scars was seen in all the patients, with 90% reporting excellent results in both cohorts. The downtime of MnRF treatment was negligible (<24 hours) whereas it was an average of 4 days with CO2 laser. Post-inflammatory hyperpigmentation and persistent erythema were the most common complication, both observed in one patient from the CO2 group. Conclusion: Both Mnrf and CO2 lasers are equally effective in treatment of acne scars, however, patients prefer MnRF due less downtime. Cost of MnRF may be higher to patient due to the consumables involved.

Abstract:

Frailty is an increasingly prevalent healthcare problem in today’s aging population. Inadequate nutrition, particularly a low protein intake reduces muscle mass and has been identified as one of the main causes of sarcopenia, further contributing to frailty. Our daily protein intake required is made up of Essential Amino Acids (EAA) that has been shown to be primarily responsible for stimulating muscle protein synthesis. This study aims to investigate the effects of EAA supplementation on frailty in older persons. A systematic electronic database search was performed using PubMed, identifying 10 randomized controlled trials and studies. The results were analyzed and compared to ascertain the effectiveness of the EAA supplementations on improving physical frailty status in terms of lean muscle mass and physical performance indices. Across these studies, EAA consumption were found to increase basal muscle protein synthesis, whole-body lean muscle mass and even other benefits of decreasing insulin resistance and reducing serum TNF-∂ (an inflammatory marker). Interestingly, Leucine (one of the nine EAAs) has been shown to have the greatest anabolic potential for protein synthesis. Overall, these results explain that EAA supplementation has been effective in improving physical frailty status in older adults and that there is the potential for this response to EAA not to remain as an acute anabolic one, but to be sustained over time. Future research into the ideal dosage, composition and mode of consumption of EAA supplementation can have implications and be incorporated into the treatment of frailty in the elderly.
